Family Disability Navigator

Navigating the School System and Community.

The Family Disability Navigator program provides families with school aged (3-22) children with disabilities, guidance on obtaining assessments, for children who have no formal disability diagnosis, are not currently receiving therapies and/or who have no IEP/504 plan in school to determine appropriate needs and interventions.

Qualified agencies are identified, and referrals made to connect families to appropriate programs and services.

Services include:

  • Information and Referral for Formal Disability Diagnosis
  • Planning, Advocacy, and Navigation of IEP/Plan 504
  • Navigation of Community Resources
  • Wellness Assessment and Referrals for Children
  • Support Groups and Education for Parents
  • Case Management
  • Skills Training
